
Payment for your office visit
is considered part of your treatment and is required for all services
provided. If your health insurance policy requires you to pay a co-payment
amount, it must be paid at the time of your visit. Our office accepts
payment by cash, check, Mastercard, Visa or Discover. If you are unable
to make your co-pay at the time of visit, your appointment may be rescheduled.
For insurance-related information, please click on the “insurance
link” at the top of the screen. Your insurance card must be presented
each time you come to our office. Please notify us if there is a change
in your insurance, address or phone number.
